Important Notification: U.S. Bank Substitute Form W-8BEN Required to Certify Non-Resident Alien Status

In accordance with the United States Treasury and Internal Services (“IRS”) withholding tax rules, any individual not a resident of the United States that receives U.S. sourced income must complete a U.S. Bank Substitute Form W-8BEN and return it to us in order to certify your tax status. If U.S. Bank can no longer treat you as a certified foreign person, we must withhold 24% Federal income tax on the interest paid on your account(s).

What is the mass W-8BEN Substitute solicitation?

In June and July 2023, U.S. Bank mailed a letter to customers whose interest-bearing accounts (checking, savings and CD) were opened without a U.S. taxpayer identification number (Social Security Number (SSN) or Personal Taxpayer Identification Number (ITIN)). The letter was accompanied with a U.S. Bank Substitute Form W-8BEN and a returned envelope.

Actions needed:

To prevent or stop this withholding we are asking you to complete a Substitute Form W-8BEN or appropriate U.S. tax form(s) applicable to your tax status and return it to us with using the returned envelope that was enclosed in the letter from U.S. Bank. Or please mail it to the following address.

Please do not send the form to IRS.

If you are no longer eligible to complete a W-8BEN because you are a U.S. tax resident, please provide us with your U.S. taxpayer identification number (SSN or ITN) by completing IRS Form W-9, Request for Taxpayer Identification Number and Certification. Form W-9 may be obtained from the IRS or from any U.S. Bank branch.
